Important Information:

  • As part of completing a referral to TPAG we require you to provide your signed consent to obtain and release information before completing the referral form. Upon completing the consent form you'll be redirected to the referral form.
  • If you are completing a referral on behalf of a client who is not present to sign the Privacy and Consent form please proceed directly to the Referral Form.
  • If you are a DVA White or Gold card holder you need to ask your GP to send a referral to our office and do not need to complete this form.
